2007 Subaru Outback 2.5L

Hello. My 07 was leaking coolant fast. Got it home before it started overheating. Leaking lower hose by both clamps. We removed it and put it back. Tempature was good for a little bit but began to climb again. Do I need to pour coolant directly into the radiator?
