Oil cap fell off

I would like to learn more about what could have caused an incident last summer. My 2016 Outback was serviced (basic service, including oil change) and two days later, after driving for 375 miles on highway and freeways, the oil cap fell off, which caused the oil to rapidly drain and the engine to seize. Does the oil cap falling off necessarily imply that the person who changed the oil did not tighten it sufficiently, or can the oil cap fall off for other reasons (aside from damage, as there was no evidence that the sump/oil pan had been hit by anything).

I am not looking to assign blame here. The dealer immediately assumed the blame and took care of the problem without charging me anything. I am curious because the question comes up every time this story (now a prominent element of family lore) is re-told.
